Mortgage Checklist

Below is a list of information you will need to provide to help expedite your loan application for a smooth and efficient process from start to finish.


  • Current driver’s license
  • Two years W-2s and/or 1099s (most recent)
  • Two paystubs (most recent)
  • If self-employed, two years of tax returns, all schedules and K-1s (most recent)
  • One month (most recent) bank account statements (non-applicable if Bank of Sun Prairie accounts)
  • Current home insurance policy

If Applicable:

  • Current retirement/investment account statements
  • Marital settlement agreement/divorce decree
  • Proof of retirement/pension income
  • Trust documents
  • Current mortgage statement (refinancing)
  • New construction – blueprints, plans & specs
  • If available, signed copy of the offer to purchase contract
  • If you own investment property or a second home provide copies of current rental leases and a current mortgage statement
  • If you have been in school in the last two years provide a copy of degree/certificate and certified transcripts
  • Selling present home we will need a copy of your closing disclosure after closing
