We will then process your application, if it is approved you will receive the appropriate forms that need to be signed and notarized along with a return envelope in the mail. Upon return of the executed forms, we will issue you a card(s).

I/we understand that this Debit Card will work in a TYME/ATM Machine and if I or another owner give the confidential Personal Identification Number (PIN) to another person that they will be able to make withdrawals from the account(s).
